      Relicensure Process Peoria Area EMS System Providers (EMT-B, EMT-I, and EMT-P / PHRN's) are required to re-license every four years. Below are the policies and paperwork needed for re-licensure. If you have any questions, call Carol Swank, EMS QA Coordinator, (309) 655-2626. REMEMBER it is your, responsibility to ensure that you are re-licensed! If you do not receive your license within six (6) weeks of submission to the EMS office, call the EMS office immediately.
